Prior to your appointment, we will email the necessary forms for you to complete in advance. This helps streamline the check-in process and allows us to serve you more efficiently. As our schedule fills quickly, we appreciate your timely response.

Referring Offices and Facilities
Physician offices, clinics, and healthcare facilities referring patients to our practice may complete the attached referral form below and submit it along with all relevant clinical notes, diagnostic testing, and patient demographics. Providing complete documentation helps us review the referral efficiently and schedule the patient in a timely manner.

If you have any questions regarding the referral process, our staff is happy to assist you.

Please verify your insurance coverage prior to scheduling an appointment. A list of accepted insurance plans is provided below. We regret any inconvenience if we are not participating with your specific plan.

While we make every effort to accommodate patients whenever possible, we do not offer self-pay options unless your general eye doctor contacts our office directly to arrange the referral.

Please note that we do not accept Medicaid, including state Medicaid plans or Medicaid plans from other states, as we are not contracted with any Medicaid programs.
